I have exchange online as a part of MS365. I have set up a fresh/new local copy of Outlook on my fresh/new PC -- and have set up a fresh/new iPhone and an iPad as my mobile devices.
Most everything is working beautifully together. The one problem is CONTACTS.
I imported a have a good, single list of contact into my local copy of outlook. There are no duplicate entries there. I have set those up to sync through Exchange online. And there are no duplicates when I access my account online. However, on both my ipad and iphone, many entries show up twice, three time . . . sometimes as many as 5 times.
I have tried to isolate the problem (e.g., are there separate entries for every phone number associated with a single contact). But there does not APPEAR to be a pattern.
How do I solve this problem?

The contacts were saved in your Outlook and Exchange online are working fine. And you met a problem how to synchronize these contacts into IPad and IPone without any duplicates.
If my understanding is correct, on my side I can give you my private using experience how to prevent duplication, even how to delete existing duplication on both devices you mentioned.
> To eliminate the existing contacts go to Info on the iTunes > unchecked Sync Contacts with Outlook. It'll ask if you want to delete then. Say "Yes" and the contacts will be gone in both your two devices.
> At this point you can resync. In iTunes go to the Info tab and scroll to the bottom. You'll see under "Advanced" a set of options under the heading "Replace information on this iPhone". Check the Contacts box and re-sync. This will overwrite all of your contacts with what's on your Outlook, and then save this info so the subsequent sync's will work correctly.

This answered my question and resolved the issue -- at least temporarily. I still don't know what caused the problem to begin with -- and whether to expect it to occur again. But the fix was quick and simple, so I guess I could do it again if necessary.
I have looked some reference from Apple support community. The possible reason why you got duplicated contacts both in your IPhone and IPad is you haven't accepted merge contacts when synchronizing between Outlook and your devices during configuration. I am having the exact same issue, however we are not syncing with MobileMe, iCloud, iTunes, only, only exchange. First we flush all devices and verify in outlook 4303 contacts, then sync the iPad 2 and iPhone 4. Wait one day, and we have doubles, tripples, etc on both devices, then they start syncing back to outlook and we have multiples on Outlook 365 too. This is a real problem, and MS and Apple need to get together to fix this ASAP.
When you set the synchronization between Outlook and iPad2/iPhone, did you accept the merge contacts?
If you don't allow merge contacts during configuration of synchronization, it is high possible to cause duplicate contacts.

I'm glad to hear that it is not just me. For what it is worth, here's a bit more.
First, I think that you and I started the same, with only ONE set of contacts -- resident in outlook on a computer. There is no chance in either case that the duplicates are coming from "pre-existing" contacts that were resident on the iphone or ipad. Right?
The differences in your experience and mine (to date) are these. 1. I never ended up getting duplicates BACK on the main original outlook file. Thanks goodness! And 2. Once de-linked and re-linked my devices, the issue has not come back (yet).

Ok -- my iphone has started to duplicate entries again. Anyone have a "stable" solution to this problem?
I though I was alone in this one!!!!.... I had exchange 2008 and the contacts started to duplicate on my iPad after the IOS 5 update BUT they were reflected on my outlook, so everytime I had more and more!!!!, so I changed to Office365 thinking my exchange was the problem, but NO, again, duplicates started to come BUT this time they were not reflected on my outlook (this is a very good thing).... anyone has tested this on an iPad not running IOS 5?, or found a solution?, or submited this issue to apple?,
I have the same problem as HMalloryC. There is only 1 account on the iphone, which is Exchange. I have never plugged the phone into iTunes or Outlook. Just Exchange thru Activesync.
I tried Martin's suggestion but the suggested solution relates to Mobileme which is the old version of Activesync and I don't get an option to merger, I only get an option to delete the contacts already on the phone.
Also, I called Apple on this today and they say it is an ActiveSync problem and I should contact Microsoft. I'm using office365 Exchange server 2010, Verizon iPhone 4s, IOS 5.
I'm not sure what to do anymore.
Microsoft problem??... JA... I love these Apple guys, they think Apple does everything right and they don´t have any errors.... I can list prently of errors that they have and all of them will say it´s a Microsoft bug. So, What to do?, sit and wait for someone to fix this?, :º(

I have looked for some reference about this issue point. And many people faced this issue after they upgraded to IOS5 and use iClound. It seems not only related to Exchange, but also for some other mail services providers.

It seems not a particular issue that it is occurred to Exchange. So I think the better way is you can post questions or find out solution in Apple Support Community.
I am pretty sure after the IOS 5 update I started to have this issues..., I have never used iCloud to sync my contacts, I only use iCloud to find my iPad.. In this account I have:
One iPad with duplicated contacts, one Windows Outlook working fine, Two Mac books with Outlook for Mac, Address Book and iCal working fine.
In this forum: community.office365.com/.../84539.aspx you can see that "David" also have the same result of duplicates using Mobile Me, so why does apple thinks it´s nothing wrong?.
I live in Mexico, and Apple support won´t do me any good.
Does anyone one uses blackberry cloud service as well?
No -- I do not use "iCloud". I have iCloud loaded on my apple products -- and I have it turned "on" for "find my phone". But that is all.
The contact issues is solely set up on exchange.
Yes -- all of this is after IOS 5 -- but I didn't use any of these devices prior to IOS 5. (not an upgrade--just how i started).
