You can do the following steps to create a web database is based on the built-in template, click Site Actions>New Site>Browse All>Choose Categories->Web Databases, see below snapshot:

Currently, there is no built-in template to achieve the goal directly.
However, we have several customizable templates in SharePoint Online team site, such as “Team Site”, “Group Work Site”. These templates may work around the requirement.
Meanwhile, I’d like to confirm the following things with you:
1. It is team site or public-facing website that you want the template for.
2. Do you have current IT support system?
3. Are you using cloud-based only environment or you need to integrate with your own third-party IT support system?
Have you looked at the web database templates, there is one for issues, and one for assets, both should work for you.
